At Human Touch Home Health Care our medical social worker works directly with patients and families and counsel them on how to deal with emotional and social problems that are related to the patient’s medical disorder or disability. They are trained to link the patient with community resources. They advise patient and family on following medical recommendations and teach them to use community resources to resume a normal or active lifestyle – without being inhibited by their medical condition.

The medical social worker will assess the psychosocial functioning, environmental and the support needs of patients and families. This may include connecting patients and families to necessary resources and supports in the community; supportive counseling or helping to expand and strengthen the network of social supports. The medical social worker is part of the interdisciplinary team: nursing, physical therapy, occupational therapy and speech therapy.

The focus of the job is to help people who do not know where to find help.
