Human Touch Palliative and Hospice Care’s medical director will work in conjunction with the patient’s primary care physician regarding the medical management of the patient. Human Touch will encourage the patient’s primary care physician to remain involved if they are able to.  Human Touch’s medical director leads the hospice team in the development of the plan of care for the patient and is available for any after hours issues

Human Touch Palliative and Hospice Care’s registered nurses’s and licensed vocational nurses will monitor the patient’s condition and provide the appropriate care in order to maintain the patient’s comfort. From admission and through the course of care, Human Touch’s registered nurses will evaluate the patient’s condition and communicate with the physician and coordinate the plan of care by educating the family of proper care techniques and use of medications.

Human Touch Palliative and Hospice Care’s certified home health aides and homemakers are utilized to care for the patient’s activities of daily living, bathing, grooming and dressing and whatever may be needed at the time of the visits.

Human Touch Palliative and Hospice Care’s MSW’s will be available for all patients and their family member to offer emotions support and assist with any legal matters that may need to be addressed prior to the patient’s decline.

Human Touch Palliative and Hospice Care’s chaplains will be available for all patients and their family members to offer emotional support and counseling.  Our chaplains can work with the patient’s clergy of choice.

Human Touch Palliative and Hospice Care’s bereavement coordinate will be available to assist the patient’s loved ones with their grief and loss.

Human Touch Palliative and Hospice Care’s volunteer coordinator will locate and if appropriate will assign a volunteer to provide socialization for the patient and give a temporary respite for the family.
