What is Hospice and Palliative Care?

Hospice and Palliative care involves a team approach to give expert medical care for pain management, emotional and spiritual support tailored to meeting the patient’s needs and wishes. Human Touch views that patient and family as one unit of care. Both physical and emotional care is extended to both the patient and family members. Comfort and compassion are key to hospice services provided by Human Touch Hospice and Palliative Care.

Where can care be provided?

A hospice is not a setting but a model of care for those who have a life limiting disease.  At Human Touch we are able to provide care in any setting that the patient calls home.  Care can be provided in a private residence, skilled nursing facility, assisted living facility and a board and care setting

Who is Eligible?

In order to be eligible for hospice care, the patient’s physician in conjunction with Human Touch’s medical director will need to certify that the patient has a life limiting disease process, and meets the hospice care criteria.  Above all the patient much chose to received hospice care and not seek aggressive or curative treatment.
